Registration Fee Inclusions

The registration fee includes admittance to the forum, program materials, and all food functions as specified on the agenda.

Registration fee does not include hotel accommodations

For meeting registration fees to be refunded, written notice of cancellation must be received by Hemedicus by 4/1/12. The amount of registration fees remitted, minus a $75 administrative charge will be refunded.

Substitutions are permitted with written notification.

After 4/1/12 no refunds will be made.

A block of rooms has been reserved for registrants at a special meeting rate of $179 per night exclusive of applicable state and local taxes. You must be registered before being assigned a room, which will be available on a first-come basis.

The conference rate is guaranteed for three days prior and three days post the official conference block and subject to hotel availability.

Do not call the hotel directly. Early arrivals and late departures will be based upon availability. Sleeping room charges will be guaranteed by the credit card information provided. Your credit card will be charged by the hotel upon checkout. Room cancellations must be made through Hemedicus no later than 5 business days in advance of your scheduled arrival date. Otherwise your credit card will be charged for 1 hotel night.
